Stoughton Hospital and other area hospitals and healthcare facilities are celebrating this day which is dedicated to educating and empowering the public and providers about the importance of advance care planning. Advance care planning can ease the stress for you and the people you love at a time of crisis.
To celebrate National Healthcare Decisions Day, the social workers at Stoughton Hospital want you to learn about advance directives. These are legal documents such as a living will or durable power of attorney for health care, which allow people to convey their decisions about end-of-life care ahead of time. Advance directives provide a way for individuals to communicate their wishes to family, friends and health care professionals - helping to avoid confusion if they are unable to make health care decisions for themselves.
